BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2016-218857 DESCRIPTION A previously healthy man, aged 32 years, was admitted with severe epigastric pain, vomiting and low-grade fever since the previous week. On physical examination, he had moderate abdominal tenderness. Laboratory results showed mild leucocytosis. After a careful interview, he revealed that he recently ate sushi. An upper gastrointestinal endoscopy was performed and showed on the gastric body, a filiform parasite firmly attached to an area of swollen and hyperaemic mucosa, with its end penetrating the gastric mucosa (figure 1). The larva was removed with a Roth net and the patient’s symptoms resolved immediately (figure 2). Microbiological analysis showed the larva belonged to Anisakis spp. Anisakiasis is a zoonosis caused by nematodes parasites of the genus Anisakis. It is caused by the consumption of contaminated raw or undercooked fish or seafood. Most of the cases were described in Japan due to food habits; however, it has been increasingly recognised in Western countries. 3 Patients can have allergic symptoms like angioedema, urticarial and anaphylaxis. Gastrointestinal symptoms include abdominal pain, nausea and vomiting and complications like digestive bleeding, bowel obstruction, perforation and peritonitis can also arise. Patients can have a low-grade fever. A severe leucocytosis can be present, but peripheral eosinophilia is rare. Three clinical patterns of the gastrointestinal tract involvement were described and all of them can mimic an acute surgical abdomen: (1) gastric acute form in which endoscopic removal of the larva is a curative treatment; 2 (2) intestinal form with an acute or chronic presentation, a more challenging diagnosis and classically surgically treated; 3 (3) ectopic subtype with peritoneal cavity involvement. 3

Intestinal Anisakiasis Treated Successfully with Prednisolone and Olopatadine Hydrochloride
The clinical characteristic of gastrointestinal anisakiasis is severe abdominal pain after eating raw fish. Intestinal anisakiasis is more uncommon than gastric anisakiasis. Most patients with intestinal anisakiasis need hospitalization because anisakiasis can cause intestinal obstruction, ileus, peritonitis or intestinal perforation. We report a case of intestinal anisakiasis. A 43-year-old wo...

A 54-year-old man complained of lower abdominal pain and diarrhea. Laboratory examinations revealed an elevated serum level of C-reactive protein (4.45 mg/dL) and an increased percentage of eosinophils (7.1%, normal; 3-5 %) whereas white blood cell count was normal. He had ingested raw fish one day before the onset of disease. Abdominal computed tomography (CT) showed wall-thickening of the ile...
متن کاملCommon Symptoms from an Uncommon Infection: Gastrointestinal Anisakiasis
Clinicians can be forgiven for thinking of anisakiasis as a rare condition low in the differential diagnosis of abdominal pain. Gastrointestinal anisakiasis is a zoonotic parasitic disease caused by consumption of raw or undercooked seafood infected with nematodes of the genus Anisakis. Even though the reported cases indicate that this is a rare disease, the true incidence of the disease could ...
متن کاملThe Association of Anisakiasis in the Ascending Colon with Sigmoid Colon Cancer: CT Colonography Findings
The association of anisakiasis of the colon with colon cancer is rare and difficult to diagnose. Only one case of this type has been reported to date. In this study, we report a case of synchronous colon cancer and colonic anisakiasis. A 50-year-old woman was admitted for abdominal pain, and a volume-rendered surface-shaded image of CT colonography (CTC) revealed a concentric narrowing in the s...
متن کاملThe Clinical Characteristics of Anisakis Allergy in Korea
Anisakidae larvae can cause anisakiasis when ingested by humans. Although several groups have reported a gastrointestinal Anisakis allergy among people in Spain and Japan, our report is the first to summarize the clinical features of 10 Anisakis allergy cases in Korea. We enrolled 10 Korean patients (6 men and 4 women) who complained of aggravated allergic symptoms after ingesting raw fish or s...
